Commercial General Liability
- Completed Acord Application or equivalent for the General Liability exposure.
- Brief description of the insured and related properties.
- Complete addresses for all locations (not mailing address)
- Details of property including age, fire, life, safety, when updated and occupancy rates. Supplements or detailed spreadsheets are welcomed!
- Currently valued loss history for the past five (5) years, three (3) year minimum.
Commercial Property
- Completed and signed Acord Application.
- Full Named Insured and any additional named insureds
- Full address of each location (not mailing address)
- Occupancy type and rate.
- Construction Type.
- Number of buildings in the TIV. If more than one building, how far apart. Per location values.
- Age of building(s)
- Square Footage for each building(s).
- Number of Stories for each building(s).
- Sprinklered status
- Any other alarms and if they are central station or local
- Protection Class
- Breakdown of values, i.e. building, contents, Business Income, extra expense, etc. for each building(s).
- Current or proposed co-insurance level.
- Five year loss history, currently valued. If a loss occurred, a written explanation is required for losses over $25,000.
